We identify with you the adjustments that can support independence, safety and daily habits: fall risks, adapting the space, assistive devices, medication management.
04Transitions and coming homePreparing the move from one setting to the next.
We prepare the transition, coordinate the services needed, and ensure continuity once the person is back home: temporary housing, private residence, long-term care.

Who we are
Tandem grew out of one conviction: when we act sooner and the right people work together, more possibilities stay open.
Our approach rests on collaboration, complementary expertise, adaptability and prevention. The person and their objectives stay at the centre, and the plan evolves with their reality.
Émilie Tétreault, founder, was director of care in a private seniors’ residence. Around her works a multidisciplinary team that brings together the expertise each situation calls for.
