Our services

Here is what we actually do

No jargon. Every mandate starts the same way: understanding your situation before proposing anything.

01Full in-home assessmentA complete picture, and a written plan.

Health, independence, housing, safety, family network, financial resources. You leave with a document, not an impression.

02Service coordinationWe activate the right people, and check that it happens.

CLSC, family doctor, occupational therapy, home care, meal delivery, adapted transport. We make the calls and the follow-ups for you.

03Support for living at homeStaying home, with the right things in place.

We identify with you the adjustments that can support independence, safety and daily habits: fall risks, adapting the space, assistive devices, medication management.

04Transitions and coming homePreparing the move from one setting to the next.

We prepare the transition, coordinate the services needed, and ensure continuity once the person is back home: temporary housing, private residence, long-term care.

05Caregiver supportYou stop carrying the situation alone.

Clear information, mediation of family disagreements, respite, healthy limits. Caregiving lasts years: it has to be sustainable.

06Advisory for organizationsFor residences, intermediate resources and employers.

Practice review, team training, change management, improving continuity of care.

How it works

Five steps, and you always know where you stand

Prevention guides every step. Nothing is left to improvisation.

  1. Understand

We carry out a full assessment of the person, their environment, their risks and their real needs.

In practice: an assessment meeting, then a written report given to the family.

  2. Secure

We quickly put in place the measures needed to reduce risk and prevent avoidable situations.

In practice: immediate dangers come first: falls, medication, nutrition, isolation.

  3. Stabilize

We coordinate the priority interventions to establish clinical, functional and psychosocial stability.

In practice: the situation stops getting worse, and the family gets a workable routine back.

  4. Mobilize

We activate the resources, partners and care pathways best suited to the situation.

In practice: we make the calls and the follow-ups for you.

  5. Maintain

We support lasting continuity, aimed at safety, independence and quality of life.

In practice: follow-ups planned and adjusted as the situation evolves.
